Yep, a bad center seal would do that.
The seal can be replaced with an aftermarket one (two piece) fairly easily.
The fan can be replaced with a used one, or just make sure that it is balanced by breaking off another blade on the opposite side.
The washers were probably an attempt to make the wrong fan belt fit.
